Contract Rules

This form must be signed and returned with your completed
registration form.

This .PDF document summarizes
the Center Stage rates and pricing for classes.

Change Request Form
The Class Change Request
form is for adding/dropping classes from your schedule. You must complete this form
by the 25th of the current month to have your drop effective
for the following month and pay a $5 drop fee. You may not drop classes
in the current month you are in. Changes to class levels
or adding classes can be made in the current month without
a fee, but cannot be completed without this form.
